Please use the following absence note to explain why your child is absent after a period of illness. We ask that it is returned with your child on their first day back at school. You may also use this form if you are requesting to collect your child early from school or to take them to a Doctor's / Dental appointment. In this instance the form should be sent in to the Class teacher on the morning of the planned 'early departure'. NB We ask that you ALWAYS collect your child from the classroom so that teachers can be confident that your child has left the building in your care. Thank you for your co-operation.
